We are an innovative and forward-thinking practice of 8,300 patients across 2 sites; as a team we are close-knit, welcoming, and supportive consisting of two GPs and a pharmacist as a Partner.
- We have an experienced nursing team, as well as ANPs, MSK Practitioners, a Paramedic, Mental Health Practitioners, Social Prescriber, General Practice Assistant and Care-Coordinators
- We have developed an innovative approach with GPA/CC available and on hand to manage wider GP responsibilities (eConsult/tasks/insurance etc.) which has significantly reduced GP admin burdens.
- We have established a cohesive prescribing team led by Clinical Pharmacists, Pharmacy Technician, and prescription clerks.
- Paramedic and pharmacists run home visiting/care homes service alongside the care-coordinator.
- Full workflow optimisation through highly skilled admin team.
- GP appointments are a minimum of 15 minutes
